About My Business:


The wrap around vision of care delivered by HFSS is focused on the more complex and vulnerable groups in society who need specialist support to live independent lives, these individuals may have mental health needs or have a learning disability. Many of our users, adolescents’ and adults are in transition from spending time living with family, in a residential care or acute care setting and now wish to re enter main stream community. For many years we have observed the breakdown of community placements due to the lack of a wrap around service causing re admission to either an acute or non acute residential environment. Our aim is to give individuals the opportunity to live supported lives with the additional knowledge of a robust network around them. This is achieved by our service having a community hub, called a skills centre, fully equipped with life skill learning equipment such as a training kitchen, training laundry, IT suite, chill out area for de-escalation and a music therapy area with of course an activities area for games; pool, aerobics or any number of activities. The support service is coordinated from the centre, being registered with the Care Quality Commission to facilitate domiciliary care and outreach services. The domiciliary care or home care is provided to the individuals at their own home or supported living apartment/flat (which can be supplied by Trans Pennine Community Housing Association) or in the skills centre allowing life skills to be gleaned, such as basic cooking, paying of bills use of a ATM or washing of clothes, the centre assists individuals in C.V. writing and gaining entry into the work environment. While individuals are not receiving their daily support either in or out of the centre they can simply drop in to chat or to feel a sense of belonging. The service is aimed at individuals who require more than low levels of support. Some of our users have highly complex and challenging needs. We have found that with this wrap around model of specialist support individuals are increasingly likely to feel safe and secure and learn skills whilst the community placement is less likely to breakdown. Our Life Skills Coaches work with individuals to enhance existing skills and develop news skills through a person centred approach.
